It is suggested you bring:
A Photo ID (driver’s license or passport)
Your HealthSource RI username and password
Any recent HealthSource RI or Medicaid notices that you have received
Your 2014 tax return
You may also need the last 4 weeks of your pay stubs/proof of income for past month, and social security numbers/immigration documents.
